You don't normally have to take the hub off to do a disc. I did my Escort van ones a couple of months ago and all you had to do was take the road wheel off, swing the caliper out of the way (don't forget to put on new pads with a new disc), remove a circlip off one of the studs and then knock the whole disc off the studs. Essentially, the wheel holds the disc on (at least on cars and car derived vans), not sure about a Transit though.
