For more on marking an answer as the "Best Answer", please visit our FAQ.The B on the middle of the stave is played by playing low E (the lowest note you can get on a B flat clarinet) and then use your left thumb to press the register key (the key above the thumb hole). You still need to cover the thumb hole fully while pressing the register key. In order to get notes higher than B, the register key works all the way up, e.g. to play the C above that B I've just described, you play low F plus the register key.
