Thats the way it works in government nowadays. You get rewarded for failure unless,of course,you are at the bottom of the ladder and then all pooh drops in your lap.
Didn't make a very good job of Immigration and Borders either.

This wouldn't have connection with the fact that it is now next to impossible for any of us to contact the HMRC would it ?

I don't feel sorry for her at all- she will be retiring on a huge pension (and will have made sure her own personal tax queries are well and truly sorted).
As for how she got a Dame - these honours are nominated by fellow civil servants, it's the system and always has been.
