It's not about hindsight either. This sort of thing happens too often.

If people are shown to be failing in their duties they should face disciplinary proceedings. The CEO's sometimes fall on their swords but there are undoubtedly indians out there who are too complacent and don't take enough responsibility. I've worked with plenty of such people.
