The lift doors opened and closed twice before she was pushed out when it started going up and he didn't have time to pull her back in. It's a fault with the lift.
I just hope she still had enough drugs in her system to make her suffering as minimal as could be.

Ummm....not an error with the operator, but as the lift had only been inspected in August 12th, then its likely that someone must be to blame for not noticing that a fault had developed allowing this to happen.
In fact, as the lift was only inspected last week, I suspect that the inspection itself must be in some way to blame.
Lifts are not supposed to move if the doors are open.

Terrible thing to happen.
As to the cause we shall have to wait on the investigation. When I worked in a warehouse many years ago there was often a problem getting the meat in the lift. It was possible to override the door mechanism with a piece of cardboard and the was done regularly. It was also done once when the door mechanism played up. So at the moment it could be anything.
