It's just some kind of random insult which a child of four would find funny. 

You can imagine him rolling around on the floor every time he says the word, kids of this age find the strangest things hilarious!

English children are taught from an early age, like one year of age, that bodily functions and the words for them are 'bad' words. Other bad words are awarded stars on this site, as you must have read. Think pee or poo.

Toilet head is as bad as you can get.

If you recollect the film ET, in the very first scene, one child who has been told not to call his brother d+c+head, calls him p+n+sbrain and there are squawks of dismay and protest from the mother.
