I do not understand this not that bit.
it is a bout a woman who ended up with a drunkard, escaped from him. when she was with him he did not allow her to meet anyone.

In a lot of cases "not that" can mean "even though", it is a way of adding information that shows two sides of the same peice of information.
As in:
"I'm leaving you, it's not that I don't love you, it's that I don't trust you."
Your original sentence could say :
"He'll do the rounds, visit all her friends, even though she hadn't really got any left."
