Ten years ago my place of work was merged with another company and management consultants were brought in to oversee everything. The term 'going forwards' became compulsory in every communication- the best was "Going forwards we will go forwards together".
Change became transformation.
The word problem had to replaced by 'challenge'.
Every change was justified as being more 'modern'.
After a few weeks the phrases jarred but if you didn't use them you were seen as an 'old fashioned' thinker.
I was glad to leave.

The company now has major problems, it has gone backwards and, going forwards, will not survive more than another year.

I'm unsure what the issue is. Saying something is now understood that wasn't before doesn't absolve any responsibility. If one is to make a post problem announcement some kind of, "it won't happen again", in whatever words actually used, is something anyone would opt to put in, isn't it ? Serious failings is hardly not taking responsibility but admitting to the issue.

Will these serious failings which they should have learnt from earlier result in people being 'accountable' for their positions of 'responsibility' which they get paid very handsomely for. I very much doubt it, if they do they will go with very handsome golden handshakes.
