Where did the phrase 'Faint heart never won fair maiden originate? Cervantes? Shakespeare? Is it 'Never' or 'Ne'er'? and is it 'Lady' or 'Maiden'

The version I've always known is "Faint Heart Never Won Fair Lady" and it's been attributed to (amongst others) Cervantes from "Don Quixote".
Rabbie Burns certainly wrote: "Faint heart ne'er wan a lady fair", but he may well have been paraphrasing Cervantes, of course.
