It means somone has used a mixed metaphor.

It's just a jokey mix of meatphors - you usually 'put your foot dfown' to make a point, and deal with something 'with a firm hand', and somewhere along the line, someone put them together, and it has passed into common useage.

It's similar to 'making a mental note on a piece of paper'.
