I've never heard such a saying, but one old meaning of kidney was temperament, nature or disposition. I would guess, therefore, that it means she's got a particularly strong personality or attitude (perhaps unpleasant). Possibly much the same as she's a bit of a madam!
Kidney tastes rather bitter and perhaps the speakers were confusing it with bile from the pancreas. Certainly it means someone who's in a sour mood, perhaps perpetually.
