Morning all. 6 Down. Medical Records. C?S?????S. Appears to be Casenotes. This gives 12 Across - Reed instrument - 4 Letters ?N?E and 15 Across - Have a kick at goal - 5 Letters ?H?T?. Initially I had SHOOT for 15 Across. Any thoughts/ suggestions much appreciated at this early hour.

6d seems to be causing a few problems this week. Initially I had casenotes, but then realised it didn't fit in with 12a reed instrument which I am sure is oboe and also affected 15a, which I am positive is shoot. I think 6d is actually casebooks - from that point the other answers fit in.
