I am not too happy with the flea one though as I don't see where the syringe fits in- unless it's to suck out the flea

Well I'm confused now
I suppose if you had a flea in your ear, you might need a doctor to syringe it out. Bit random, though.
I've heard the phrase, it was the doctor with syringe that threw me
