Although I have a full grid I have only figured out 8 of the 10 modifications and I'm still struggling with some of the wordplays. Nowhere near attempting the answer required to be entered beneath the puzzle yet. It's good to have a challenge though.
Yes, this is very tricky. I've got a grid that's as full as I think it's meant to be, I believe I've got the nine spurious words or phrases, but if so I have a choice of ways of entering one of the medical problems -- which would then clash with a possibility for the tenth for which, in turn, I can't link to phenytoins. It's gone into the pending tray for the moment. This week's Listener was a doddle in comparison.
I have it most complete. Three lights to enter. Have 8 of the symptoms, and have sussed the title. But have drawn a blank the last couple of times.

Shoni70, both 2a and 42a are thematic and need "changing" before entry. In regard to 2a, 40a contains the word that needs to be removed before solving. Both are subject to the sentence in the rubric relating to the length of grid entries and space available.

Aha! That time it spent festering in the pending tray did the trick, and it's now all fallen into place. A more careful reading of the preamble removed an uncertainty, and a sudden improvement in my ability to count was a bit of a help too. I agree that the title, linked with a careful reading of the preamble, is very clever.
