Rail Rai?

Tenuous at best. A Rai was a stone of value in some island cultures(eg Guam) Where the doctor come in I do not know.

If Navel/Nave is correct then I can only agree with the Winner that the clues aren’t very good but it fills a blank
Just need
Hold tightly onto this: it is currency for doctors.
Grasp/gasp? Gasp leaves GP as the first and last letters but can AS be a currency?
S is recognised as a symbol for dollar - a dollar?
Is grasp/gasp corrct?
Not sure, barry. '' Hold tightly onto this''
suggests that it's 'something' you hold on to
You can hold on to Hope?..
I thought I was very clever
Like hat or purse or rein or rail

I'm not sure we have it yet but thanks for trying
Best I could do with for 1 is:

dream / dram

"Hold on tight to your dream", a song by ELO
Dram, as well as a small drink, is an alternative spelling of drachm, symbol "dr", a unit of weight formerly used by apothecaries, equivalent to 60 grains or one eighth of an ounce.

That sounds great Ellipsis!
Thanks! Also there is a currency, the Armenian dram:

https://en.wikipedia.org/wiki/Armenian_dram

It fits, but it seems very obscure!
I agree it's obscure but the only one that really fits
Yes, I'm now sure it's correct. You could hold tightly onto anything, so better to start the other way round (start with the second one, then add a letter to get the first one):

"it is currency" (dram) "for doctors" (dr, the abbreviation of dram)
Then add a letter, dream, to give something you can "hold on tightly to" (according to the ELO song)
Well done Ellipsis! :-)
Yay!
2. Naval / aval

Navel is the orange, so "not the orange kind" is Naval
Aval is "a financial guarantee by a third party to assume the burden of a debt, especially a bill of exchange in the event of default"
I agree with TheWinner for number 3, trill/till

So I would say ...

1. dream / dram
2. naval / aval
3. trill / till
