I have completed about a third of this but are at a complete loss as to what the 3 word phrase at 23 across is. All I have is ????t????t??? Any help would be very welcome.

18d eifel (life+e) 3d another (a+throne) gives you two more letters in phrase.

Thanks Max, I think you are right with 23A. but I cant tie it in with the "treatment to be given". If you want to compare other solutions, cant guarrantee they mine all correct but some are certs, please post again.
