When you are bestowed with this honour for whatever reason what exactly does it mean?

What do you get that us mere mortals dont?

You get a big novelty key and the right to herd sheep down the main street.
It should mean you never have to pay for a drink in a bar again.
Homer Simpson got the 'Key to the City' in one episode. He left bite-marks on it, as he thought it was chocolate wrapped in foil, if I remember rightly!
QM hope you did not miss the bit in your link about Saddam Hussein being given the key to the city of Detroit, USA as he gave a donation of $170,000 to a church. Ring any bells with the current goverment and the carrots they dangle in front of potential donors?
I bet you Saddam wishes he could swap the Detroit Key for the key to the front door of his prison.

Dom...Dom...Dom! This is the 'People & Places' category, not the 'News' category!

I now visit the News' category of AnswerBank in exactly the same spirit as London's Victorian toffs arranged a Sunday afternoon jaunt to Lambeth, specifically to the Hospital of St Mary of Bethlehem there, better known as the lunatic asylum...Bedlam. That is, for a laugh.
Looking at a random sample of News' questions and answers from recent months, one is forced to conclude that several contributors there - no names, no pack-drill obviously! - are borderline demented, too.
In the face of such persistently benighted bigotry and monumental - though apparently blissful - ignorance of relevant facts, laughter and silence are the only rational responses.
There was a time when one could have a reasoned debate there with at least some users; sadly, the News' category is now little more than a Daily Mail' supplement and I long since ceased taking such comics seriously.

Accordingly, I'll treat your 'news' query above in precisely the same way. Now, have you anything to tell Don1 about the freedom of cities?

AAAAH bliss i got a response from the legendary QM.

Glad to know that you do visit and read the news category.
