Blacksmith's grave - Somerset?

While on holiday near Minehead some years ago, we went on a treasure hunt. One of the finds was a gravestone for a smith, containing lines like "his forge has grown cold".

I think the trip was no more than 50 or 60 miles, but (for family reasons) would be very interested to find out where the churchis and what the inscription is.

Any ideas?
