if two players touches the football at the same and it goes at the back of the net, whose name goes on the score sheet? do both names goes on it?

No-one , it`s disallowed and a throw in is awarded to the nearest player

snowyball - here is an example - two players simultaneously kicked the ball into the net. The goal was credited later to Perez after examination of the video.
