It normally just means the message has been sent with high importance.

Are the viagra ones sent with high impotence? :-)
Not always, I receive a lot of mail marked as high importance that's genuine. but spammers will quite often mark their messages as high importance in the hope people will be more likely to look at them.
Depends on what mail agent you are using.
