Uploading Sample Page

I'm in the process of applying for permission to use a quotation from a copyright work. As per the publisher's request, I'm trying to upload a sample page of a planned publication, but I must be doing something wrong because it won't work (I've never done this before). I've highlighted and copied the page via Edit; there is an Upload button next to a small box on the form, but nothing happens when I click (left or right) on the button or the box or try to Paste. Any advice or instructions, please?

There is a firm statement that the request cannot be forwarded without the sample page, though, so I may have to resort to Royal Mail with the inevitable few days' delay - unfortunately I'd overlooked this one and the matter is now fairly urgent. Thank you for the suggestion. I'll give it one more try.
I've noticed one or two other things on their form that don't really tie in with the purpose of the proposed publication, so all in all I think I'd better write a letter explaining the request properly. I hate these 'choose from the following options' boxes on pro-formas! But thank you both for your suggestions.
