I've got a pc that's connected to the internet 99% of the time. I've designed a personal website, and I've got the domain name - but what do I have to do to host the site myself? I'm using Dreamweaver MX... Any hints, tips or complete answers would be more than welcome.

it depends on what operating system you use and also on your ISP. Some ISPs let you host servers, some don't. Also, make sure you have a static IP address - i.e. the same one every time you connect. Then get a piece of web serving software such as apache. If you are using Windows, you may already have IIS or PWS which does an equivalent job (ish)
on the other hand, you ought to host your site on a server that is both specifically designed for the job, and is permanently connected. There are many ISPs that provide free accounts with hosting space. Just sign one up and FTP your files up to your new webspace. For your next trick, you need a host for your domain that will either dynamically forward WWW traffic to your webspace, or will allow you to specify records within your domain so that you can point your WWW record (website) to the server where its hosted. If you need moree help, msg me here...
