Motorola - unrecognised calls

My RAZR V3im usually connects calls fine but sometimes alerts me that I have missed a call when my phone has not rung. When I want to view who i've missed a call from it always says 'unrecognised'. Any clues why it moght be doing this/how I can make it stop?? Thanks for any help.
