How can I stop emails which are not addressed to me from arriving in my inbox?   More often than not from porn or medical sites...they`re a bl**dy nuisance.

I don't think you can.  I've contacted my ISP about them.  They just told me to delete them.  I found a way to get rid of most of them.  I opened a Hotmail account, forwarded all my mail from the ISP server end, and set it to delete all junk mail.  It's an option with NTL, but I don't know about any others.  The occasional one still gets through though.

Download a spam filter such as MailWasher.  You can get it here:

http://www.mailwasher.net/

or if you use Thunderbird as an email client it learns what you class as junk and does the work for you. and it's free too as well as being (IMHO) better than Outlook Express.
My friend and I tried mailwasher. We were very impressed at first, but soon after, the junk emails that were addressed to us via our ISPs increased dramatically.. up to 100 a day. We both use the internet a lot, and both use our hotmail addresses when registering on sites. It was taking forever to get the headers into Mailwasher, which is why I redirect them to Hotmail now.

MailWasher is working fine on my PC.  It "learns" what is spam and blocks it at the server.  Anything new is stopped before it is downloaded and you can blacklist it before downloading the "good" messages to the Inbox.

Mailwasher works great for me too.
One obvious solution is to use an email address that you don't enter to register on any websites as they'll not only send you a confirmation email but also 9 zillion spam emails. Have one email address which you don't mind being spammed to death as you only use it for these "confirmation emails" & one proper email address.
Whichever email address you use, try to give it only to people that you want email from - don't enter it anywhere else at all online.
