DVD - NTSC/PAL

We are having a DVD manufactured which will be distributed worldwide.
The question is, should it be NTSC only, or NTSC+PAL (double-sided)?
If only a tiny number of people would need the PAL version, it might be better to just have NTSC to avoid the problem of people putting it in upside down. Also, a one-sided disc is cheaper and will allow us to have a design on the other side.
It will be Region 0 (suitable for all regions).

Probably cheaper to produce Region 0 NTSC and a small number of Region 0 PAL separately, and distribute accordingly.
