Most airlines will try to take it into considerstion however they legally dont have to. Do they have the option where you can upgrade (at a cost) for extra leg room seats?
if its not possible to buy extra legroom seats, it's sometimes worth getting to check in slightly earlier than expected and explaining your problem to the check in staff who will once again try to comply with your request.

bulk head seats and exit seats are not the same thing, people get confused at this,, bulkhead seats are given priority to those people traveling with babies as this is were the bassinet (baby cots) are.
if u r not fit or able bodied u will not be given an exit seat and a doctors letter will def not give u an exit seat on an aircraft no airline would and will give u an exit seat with a doctors note/letter saying u need an exit seat due to an operation or any phyiscal defect on urself.
very few airlines will upgrade u on a doctors letter, if u want extra leg room then u have to pay for business class or not have anything wrong with u and be tall.
u can always ask at the airline's ticket desk if they r doing any promotional upgrade at the airport as some airlines do this and this can be from �250.00+�20.00 airport tax (y/class �20.00. c/class �40.00 tax) 1 way or 1 sector..
