Do you need anything other than a valid passport to go to USA? Do you just fill in the Visa form on the plane?

just a valid passport and an onward or return ticket (they don't like the thought of you entering on a one way ticket and hiding out for the rest of your days!!)

If you are a British citizen you just fill out the Visa Waiver form on the place. Its very straightforward.

The only exception to this is if you've ever been arrested (whether charged with anything or not), you then have to apply for a visa which costs around 70 and can take a while to process so allow plenty of time if this applies

all u need is a valid passport, a return ticket, proof of enough money to support yourself i.e. a credit card [ you probably wont even be asked about the money } and they need to know where you will be staying, we always say the Marriot Marquis on Times Square as we stayed there once and its well known [its the hotel that Arnie road through on a white horse in the film True Lies] to Americans and people usually have short stays there i.e. weekends.Yes just fill in the form on the plane, if you are squeaky clean and dont have any convictions forget parking tickets and no tv license offences.
Make sure you have a machine readable passport - most are. You can tell by looking on the photo page, there should be chevrons and your details at the edge of the page. If you do not have a machine readable passport then you are not eligible for the visa waiver program, and need to either get a new passport or a visa. A new passport would be much easier!
