A great deal of money and luck. Don't even think about it if you have to ask on here.
The UK has very stringent rules on allowing animals into the country. Your pet will have to spend 6 months in 'Quarantine' - meaning 6 months in kennels. Not really worth it for a vacation. Probably best to find a good dog-sitter!
I'm not sure the US is part of the pet passport scheme, it's mostly island nations and EU countries.
It is tricky but it's possible. I brought my cats back with me from the States using the PETS (passport scheme. They needed a microchip, rabies vaccination, blood test and parasite treatment. They also had to be shipped in the pressurised hold not the cabin. It all took a lot of money (1000s of $s)and time to do and for a vacation I'm not sure it's worth it.
