Adding a child to passport?

I have a 4 month old daughter and I am wanting to travel by plane to another state in the United States. Do I need to get her a seperate passport or can I just add her to mine? If I can just add her to mine how do I go about doing that? Someone please give me some feedback asap. Thank you

my advice to you is get a seperate passport, because at some stage on the future, if you want to leave her behind with family and have a holiday on your own, some countries insist that both people on the passport be present, especially Dubai & Middle East. Or if your lucky the grandparents may want to take her to Orlando at some stage.
