If I have a US visa I know I can stay there for 90 days but then how long do I have to leave the US before I can return for a further 90 days etc? Is there a specific time period I have to be absent from the US?

I assume that your question refers to travelling to the USA WITHOUT a visa (under the Visa Waiver Program, with an ESTA), since the '90 day rule' only applies to such travel.

ESTAs are normally valid for two years (or until the expiry of your passport, whichever is the earlier). They are valid for multiple entries into the USA (of up to 90 days at a time). There is no minimum period for you to be outside of the USA between each visit. However, neither an ESTA nor a visa actually guarantees you admission into the USA. Immigration officials retain the right to refuse you entry. If you were blatantly trying to cheat the system (e.g. by taking a couple of paces over the Canadian border and then turning straight round) you might well find that you were refused entry.

There are those who say that the period spent outside North America (and you have to leave North America, not just the US) should be as long as your stay was before visiting again. Whilst I wouldn't take this literally, it's a good guide.

If you want to stay more than 90 days you should apply to the Embassy for a proper visa - these normally admit you for 180 days at a time and you can extend your stay - I believe for a large fee - if you apply before the 180 dyas is up.
