Just a blunder. Simon's vote took them to dead lock, Olly, imo, just forgot who the other judges voted for.

The programme's scripted to the nth degree. Even the baying cretins in the audience are pre-programmed and the faux shock/outrage/joy from the 'judges' is no better than what you'd see in primary school play.
Every facial expression has a camera there to capture it.

Just because the results were announced before opening the envelope doesn't mean the show is necessarily fixed. Someone on the production team would have to have put the results in the envelope to start with - the results don't magically appear on the piece of paper before the envelope is opened. The producer or director of the show was probably preparing Olly via his ear piece to get ready, so Olly could have heard the result before the envelope was opened, but he was forgetting that the audience at home are not hearing what he is hearing.
