"Hi. My name's <whatever>. I don't know if you remember me, but we met at <wherever> last weekend. We seemed to get on quite well and I wondered if you'd like to meet up sometime. Maybe we go for a drink or have lunch or something."
Keep it honest, keep it simple, keep it open. Suggest you meet in a public place where there's something going on - perhaps a pub with a band or something. That way, neither of you need feel awkward or threatened, and can walk away if you need to.

definately keep it simple but do it soon - it you wait any longer you will begin to look strange and like you are calling her as a last option or after thought...
and here's a lesson i learnt through personal mistake - always make the first date when you don't know the girl very well a late starting one e.g. start at 9ish at night so that if it is going really badly you can escape saying ooh look the pubs closing best be off goodbye, whereas if the date is going well you can always suggest a trip to the clubs or a late drink somewhere else... for some reason i have never had a successful first date where food was involved so avoid the resturant would also be my advice...
good luck, and tell us what she says

Nothing venture, nothing gain, a true story from some years ago, when I was in the Forces, stationed at Barnard Castle.
A friend met a girl, lost her phone number, all he knew was her name, and the area she loved. He enlisted the aid of the local police, and they toured the area with him calling her name from a loudspeaker. it worked, and he's been married to her coming upto forty years.
You have her number, call her, or you'll lose her.
