Failed Discharge

Does anyone know the protocol for a failed discharge from hospital please.

Do the ambulance crew have a special report proceedure?

need a bit more detail

tried to take a patient home and found the address didnt exist ?
or what ?

just didnt come and get the patient ( too busy )

came for the patient but was too ill to go home

came for the patient, took them home but the patient was readmitted within 24 h to the same or another facility

Sorry, meant a patient being sent home from hospital when clearly unfit. Declared a Failed Discharge and returned to hospital the same day for the same condition.

no the ambulance wont keep records of this whyshould they ?

readmitted to same hospital or somewhere eelse ?

anyway contact PALS at the discharging hospital and say you wish to complain,
have an explanation
demand to have an internal review about what went wrong
as if a critical incident sheet was made out
and demand that this is entered in the hospital stats
( readmission before thirty days is - as a quality indicator I think - but that may be ops only )
