Joints nearest the knuckles, one on each hand are becoming stuck at right angles and have to be manually straightened out. Can someone suggest the best device to keep them straight particularly at night.

Have you been seen by an occupational therapist? Ours at the local hospital do hand therapy that may ease your condition and also provide splints. May be worth seeing if your G.P. can refer you into a similar service (or in some locations you can self refer, don't know what your local policy is).
Just in case it's not been considered, OH has suffered 'trigger finger' twice now (similar symptoms) and each time it has been cured by a sort of lubricant injection.
