a few remaining.

Some clues have an extra word.

5d. Unhappy sentinel, one could have been soothed by these (8)

6d. 6 Lager joint with German crew showing no constraints (5)

34a. Devious type harbouring abject idiot eastward of Perth (6)

39a. Way bathroom fitting turns? (4)

What is the 4th letter of 16a?

The three blank cells are in one row. Is that correct?

Thanks.

5 Just an anagram
16 A - a P, from the anagram
6D. Lager is extra. Joint, carpentry, is the definition. The 'with German' gives the first three letters, 'crew showing no constraints' means removing certain letters.
34. A common three-letter devious (slithering) type, with a three-letter 'idiot' inside. Abject is spare.
16a 4th letter is M -
'acre' is spare - anag PERMIT T(he) MESS anag ['after he's gone'].
Yes, 3 blanks in one row, hankir.

Thanks NAC.
16. Same thing. I am not sure wht is the anagram of. The first five letters are STI?P.

I got 5d, 6d, 34a, 39a.
Still not sure of 9a and 10a.

Clockwise begins at the first cell on the top row?

I have 16 extra words. The last few give me a girl's name.

Hankir, you missed PM's post, just a couple above. I'd forgotten to mention removing the Acre.
9d (as there isn't a 9a) EMI is extra - take the 'p' off the start of 'satisfy'.
10 alcoholic drink is 3 letters beginning 'k' - answer a revolutionary Russian.
You're right about the name (but I think the female version has the ...ley, ending - this ...lie one is definitely male).
...and clockwise begins 3rd cell of last row.

Thank you ProfessorM. (and NAC)
I did miss your earlier responses.

I tried anagram solvers for STIM... at first . They do not recognize this device.
Chambers Word Wizard is on the fritz for a while now.

My extra words are 13, 14, 16, 23, 31, 34, 40, 41, 1, 4,6,9,11, 21, 29 32, 35.

8a could be the 18th one. First word extra,
answer 3 letters for nothing + two for see reversed.
TCD does not have that word.

Last letters in 28, 29, 35 give me INE with the 8 letter girl's name.

You have a few wrong - there are no extras in 40a 11d 21d + 32d.
There are in 8a 30a + 36a ('nightmarish').
...having said that, there's no real justification for 'first' in 21d (and there are a couple of other dubious candidates both for/against - I don't know why setters have to mess around with us in this way!).
...it's the 2nd word in 8a that's extra - 'nothing' + 'see(!)' = LIN + LA [reversed] 'on reflection'
(sorry) - NIL+LA reversed
