Please explain these to me .
1. Using the right approach, making cuts in what's planned. Ans cross sectional. I can sort of see it but not entirely.
2. Take vigorous action about swelling. Ans Goitre. I have been unable to find any support for "go it" meaning "take vigorous action". I'm assuming re is about.
3. Fall for it , at first . Ans Original sin. Again I sort of get it but I'd like a clear explanation.
4. So that is fundamental. Ans Prime (fundamental) but can't get the "so that is"
5. Verdict letting you loose , free, finally released . Ans not guilty. (First clue I solved) but just can't see the "free, finally released" part.

number 4 is a puzzle.
The only way I can see the 'so that is' fitting in is that the 'that is' would give the i.e. bit but that still leave's the PRM.
Also i and e are the 3rd and 5th letters of PRIME and 3 and 5 are prime numbers...
Maybe the clue followed on from another one which referred to a prime number of maybe the clue number was a prime number.
These are certainly weirder than your average Times clue nowadays
