Morning, I have completed grid from the clues, have the four unclued entries and the phrase from the across clues but am at a total loss as to next steps. Any hints are much appreciated

The words at the top describe a MD associated with a 2-word thing ("feature") - and they are located "over" it. Sunilarly, the lower two MDs are "under" something. Then, there's an "ideal" MD between these two extremes (needs 3 letter changes)

hankir - 'terms' means last letters of... for 5d
40d - S()econd + the first half of your answer at 19a (places where you would stay to take the waters, could be another way of writing the def).

What happens to two blocked cells?

Also what is Ideal MD?
Refers to doctor? Some show in UK?

Each blank cell contains one letter - reading across they give two common phrases to do with the two emotions (????) ??? ???? and (?????) ? ????? [the two bracketed words refer to their positions in the grid - they're not spelled out).
For the title letters MD, they're parts of the word M??D (acting as the outside or f.... of that word) the 'ideal' is said to be in the middle of the two extremes given at the top and bottom of the grid.

A part of the fourth line is in one of the rows.
I am not sure what two entries have to be changed.

I too have completed the grid, including the two barred off squares. But the origin of the ideal MD eludes me despite much grid staring. I didn't know a poem was involved. Given that this is a harder than average IQ (I think) it would have helped if the preamble had said as much, though I suppose the top line is rather poetic.

I think 'origin' refers to the body's physical state as a whole - one that leads to the required emotional state (not a published work on the subject, if that's what you're thinking... but who knows?).

Thanks, Prof M, but someone did say he had the poem, with the fourth line in the grid, which implied to me that a poem had to be identified to complete the puzzle. I'll have another look.

The preamble's mention of 'hints among the clued entries' could suggest a written work - two entries suggest a possible verse form, three more suggest an author - but there are so many possibilities (including quite a few with the altered entry as their titles) that I didn't bother looking very far. My curiosity was satisfied when I stumbled on a (pretty loose) connection with a quote that includes the MD word - but it isn't in verse.
If the setter wants us to track down a specific work then highlighting the author should be required, and more information given to direct the search - 'hints' is just too broad a term, and searching for something you're not sure is even necessary soon becomes irksome (I think).
