I need to know if the hospital receptionists are aware that a patient has granted LPA to his representative/spouse/carer.
Does it come up on the screen during the booking in process?

For more on marking an answer as the "Best Answer", please visit our FAQ.It occurs to me that, whatever the hospital's policy on disclosure might be, the information that an LPA exists can't possibly appear on any screen unless the hospital authorities have been made aware of it in the first place.
For example, I signed the paperwork to help my friend set up an LPA. However I very much doubt that it's ever occurred to him to tell his GP, or the hospital, about that LPA. So there's absolutely no way that a hospital receptionist could become aware of it.
