All our duvets are coverless, simply because we don't use duvet covers any more.
We have a bottom sheet, a top sheet, then a duvet. Makes life so much more simple when changing the bedding. The duvet is, in a way, just like an eiderdown or comforter. All our duvets have very soft covers, sort of peach skin (or something like that), not expensive but very comfortable and snuggly, and look good.

I get the top sheet to clean it from 'body' type dirt but surely you sometimes get mess/dirt on the actual top surface of the duvet as well - the odd drop of tea, cat hair, make-up, anything really. I'd expect them to look quite grubby after a while of no cleaning.
I used to have them covered and used without a top sheet when I was unattached and allowed to do things properly. Then I was informed that wasn't right. Thing is, the top sheet can be a pain when getting up at night to go to the bathroom and then trying to get back. Usually it's ok, but occassionally one ends up the wrong side, or with one leg under and one over. It was never an issue when there wasn't one there.
