My ban is up next week. What will I have to do about gaining a provisional licence and passing my test?

If you had a provisional licence at the time of the offence, I'm surprised that you've not already received a form. These are sent out to licence holders 56 days before the end of the ban; I would have thought that this would apply even if you only had a provisional licence.

There's some information about this, here:
http://www.direct.gov.uk/en/Motoring/DriverLic ensing/EndorsementsAndDisqualifications/DG_402 2491

I note that this appears on that web page:
"If, for any reason, you do not receive the reminder application forms to apply for your driving licence, you can use the D1 application form, which is available from DVLA form ordering service, and Post Office branches."
So I recommend getting hold of form D1 from the Post Office and reading the information in booklet INF1D.

PS: If you're confident that you're ready for your driving tests, you can book online using the following link. (The link won't be 'clickable' because it's got an 'https' address, instead of an 'http' one. Just copy it and paste it into your browser's address bar):
