If you use your debit card in a hole in the wall outside a bank in Portugal do you get Euros out and not escudos?

yep, euros.

i've not seen escudos in portugal for years now.

Portugal use the euro now, so would be euros

The escudo ceased to be legal currency in Portugal at the end of February 2002. ATMs dispense euros.
