Have you thought about local italian Cookery Courses, or they run some great ones in Tuscany but would exceed your budget.
Does it have to be an Italian based present? If they have a garden then how about a bird table or feeder? You can also buy the food to on on/in it, and possibly some binoculars and a bird guide.
100 M&S voucher ? bit boring, but you could take her for lunch at an Italian restaurant and afterwards she could go and choose something.

thanks for your ideas. done the meal thing, garden thing, trips to italy thing, binoculars for father in law thing. we dont live near them now and they dont live near the shops. this is a real pain in the proverbial. im at a loss as you can probably gather??!!!!!!!!!!!!
I always think a really luxury version of an every day item is a lovely gift.
How about a really nice designer purse - you know, a good name like chanel or with the italian theme cocinelle perhaps. It's something she wouldn't spend that money on herself but a good purse is a much treasured item.

I bought my mother in law (or as I like to call her, the leader of the opposition) a new chair for her birthday, I hope she lets me plug it in soon.
Does she have many grandchildren? or how many children does she have? maybe get a up to date family portrait done? but im gonna guess you will say done that too?
What about trying to hunt out a particular photograph of her favourite trip to italy and having it put on a canvas. We done this for my Dad who used to sing in his younger days and had an old black and white photo it was about 45 years old but it came out fab.
Or a statue for the garden ? a sun dial would be quite nice with a little plaque inscription.
Lucky for you that you seem to have a good relationship with her, lucky her having you!
good luck
