1. I may be wrong but I think it's Durham cathedral ..from the romans ..on a statue ..Ely and Winchester have them but I have feeling that Durham is the oldest .
2 Named after River Phasis in Colchis (?)
3 Fairly sure it was the 50's
4 5 (2 triangular ends + three rectangular sides)
Prism: As I remember a prism can have an infinite number if sides
