Does improvise mean to perform and notate music at the same time?

Improvisation is basically the art of making it up as you go along. It is much used in jazz and in some extent rock and blues. Some of those wonderful guitar solos that you hear from the likes of Jeff Beck, Clapton, Richard Holgarth, Jimmy Page, Frank Zappa and the like are improvisation. Notation don't come into it - those solos are not written down, just played at the moment. The verb to improvise can be used outside the field of music eg 'I don't know how to do this' 'Well improvise then'
