Do you put all your school qualifications on your CV?

I think it depends on how old you are. If you're relatively not that old & only left school say a minimum of 10 years ago then I'd include them, but if you've done courses/degrees etc... then those qualifications are what they would be more interested in, especially if it's hands on experience. If there is an on-line application process/form it may not require school qualifications. You only need to include the grades that are worthwhile - maybe nothing if it was counted as a fail (include grades A - D but don't include anything lower).

I arrange my c.v. so that an employer can see, at a glance (by reading the headings in bold type), that I've got 10 O-levels, 3 A-levels, a university degree and qualifications in teaching and journalism. I include the details as well, so that they're available if the employer is interested, but they're not as prominent. (I don't bother with things like grades, dates, examining boards, etc. I simply list the subjects)

It's then up to the employer to choose whether to get the key information by scanning the document or whether to read all of the dull details.
