Nineholes is an anagram of He's in one +l (line) and means a difficulty.

Quite how George W fits in, other than being in a fix, I'm not sure. Someone will no doubt explain.

I think it's a heavy-handed way of indicating that the term, in the sense of difficulty/fix, is an Americanism. I can't see any other connection.
Surely it's no different from the way many crossword setters refer to "Jock's something-or-other" when what they mean is that it is a Scottish usage. In this case, as already stated, it's American. That's how I read it, anyway.
Since nineholes is a golfing term, could he be in a bunker(fix).
