You download the games as the website says, put some files onto the Micro SD card (which acts as a sort of Operating System), put the games on there, too, and basically you have as many NDS games as you want.
The ROMS are basically the games, but without the cartridge.
The games range in size - usually from about 8Mb to about 128Mb....dont think ive seen any bigger than that.
So, if you have a 2GB Micro SD card, you can have, roughly, about 40 games on it!
Not that i condone this sort of thing, of course! :)

Seems everyone has a R4 at the moment. Can't see these being legal to buy for much longer?
You can get the kit from Amazon, search for R4 revolution. There is also a make called TT. There basically a DS cart with a micro SD slot in back.
You get a kit, which normally includes a R4 card, USB stick reader, 2GB micro SD and application disc.
It's far too easy and must be really hurting Nintendo. Just download NDS files copy across using the reader.
But remember that this is illegal.
