I have 21 across as Tiramasu. But why? the only letters I can confirm so far is the i and the u.

clue:

Sweet french husband heads west in tatty suit. Its the rama I don't understand.

Spelling is incorect - tiramisu
mari - french husband ( heads west means backwards ) inside anagram of a suit

This 'sweet' quite regularly gets incorrectly spelled as 'Tirimasu' rather than 'Tiramisu'. Ans is simply (reverse) mari (Fr for husband) and place in anag. of suit.

As for 22d that's simply 'cleanliness' with the answer to 5d removed and the remaining letters rearranged.
