They should at least come out to check her BP and baby and see if shes dialating and if so, how far. If she is 4cm, established labour, she should insist on the midwife staying with her and asking for gas and air to help with the pain. With my first pregnancy, my wayers didnt break until after I had had an epidural and with my second, they didnt break! After baby was born the midwife said, so wheres your waters then? lol
How long are her contractions lasting and how far apart are they. Also, how far from the hospital is she?

He really shouldnt as they can be difficult to use. Some can have trouble finding babys heartbeat even though everything is fine and well. It has to be in the right place to pick it up and can be hard to do. He hasnt left her alone has he?
Tell her to keep ringing the hospital every 10 minutes until they send someone out or until they allow her to go in. Is there another hospital nearby she could go to?

if they have internet access this may help get an accurate timing on contractions http://www.contractionmaster.com/
a bath is good pain relief in early stages (got me to 8cms).
Is there another hospital nearby? she could call them and see if they can take her. Use the excuse that she is at a friends nearby if necessary.
Or call her on-call midwife and see if she can get a home visit
