It may be that matters have changed since I used to do this but the procedures I used to have to go through were: Firstly, you need to make a formal complaint to the Complaints Handling Partner and ask him to invoke his Rule 15 Complaints Handling Procedure (unless you have done this, the LCS will not entertain your complaint).
Then you need to write to the LCS - further details here

However, if you are alleging negligence, the LCS will not always get involved.
